Job summary

CAMBA is seeking a Case Manager in Brooklyn, NY to maintain client relationships, ensure confidentiality, and navigate health and social services. The role reports to the Program Manager and is based at 19 Winthrop Street, Brooklyn, NY 11225.

Responsibilities include outreach, intake support, data entry, and coordinating with referral organizations while ensuring program compliance and client advocacy for self-sufficiency. Competitive benefits are provided.

Qualifications

• Associate or bachelor’s degree and one year experience in case management or related social service field.
• High School Diploma or General Equivalency Diploma and two (2) years’ experience in case management or related social service field.
• Sufficient education and technical expertise to comprehend written and oral instructions to accomplish tasks, and to document accomplished tasks, and three (3) years of experience in case management or related social service field.
• Bi-lingual in Spanish

Responsibilities

• Maintain professional relationships with clients and client confidentiality.
• Practice Universal Precautions/Standard Protocol & Procedures.
• Comply with all Federal, State, City and CAMBA policies to protect health information privacy.
• Prepare marketing materials for the program.
• Conduct outreach activities to market the program to the community to recruit new clients and re-engage former clients.
• Follow-up with clients and with referral organizations regarding client contact and progress with referral.
• Input client data and client progress information into automated database.
• Escort clients to appointments and document progress notes.
• Create and maintain client files and make copies of all client documents.
