Job summary

CAMBA in Brooklyn, NY is seeking a Case Manager to provide community-based case management and health-related services. You will build relationships with clients, maintain confidentiality, and support them through barriers to self-sufficiency.

The role includes outreach, data entry, and coordinating with partner organizations. Minimum qualifications include a degree with relevant experience or equivalent, plus bilingual Spanish ability.

Qualifications

• Associate or bachelor’s degree with 1 year case management experience or higher.
• High School Diploma with 2 years case management experience or higher.
• Three years of case management experience acceptable.
• Bi-lingual in Spanish is required.

Responsibilities

• Maintain professional relationships with clients and ensure confidentiality.
• Follow Universal Precautions and standard procedures.
• Comply with Federal, State, City and CAMBA privacy policies.
• Prepare marketing materials for the program.
• Conduct outreach to recruit and re-engage clients.
• Follow up with clients and with referral organizations about progress.
• Input client data into the automated database and maintain files.
• Escort clients to appointments and document progress notes.
