CAMBA is seeking a dedicated Case Manager at 385 McDonald Avenue, Brooklyn, NY 11218. You will establish relationships with clients, perform intake and assessments, and coordinate services to move families toward stable housing and self-sufficiency.

Responsibilities include maintaining confidential records, connecting clients with resources, and following up on progress and outcomes. Bilingual English/Spanish is preferred, with a Bachelor’s or equivalent experience in social services.

Qualifications

• Bachelor’s degree or four years of relevant experience.
• Bi-lingual English & Spanish preferred.
• Experience in case management or social services is a plus.

Responsibilities

• Establish and maintain professional relationships with clients while upholding confidentiality.
• Review documentation to verify eligibility and maintain client files.
• Input client data and progress notes into CARES database.
• Conduct initial intake and psycho-social assessments of clients and families.
• Develop and revise independent living plans with clients to meet goals.
• Identify and connect clients with community resources and housing options.
• Escort clients to appointments and complete benefits applications as needed.
• Monitor progress and follow up with clients and referral agencies.
