Qualifications

• Associate or bachelor’s degree with one year case management experience, or high school diploma with two years' experience.
• Three years of experience in case management or related field.
• Bi-lingual in Spanish.

Responsibilities

• Maintain professional relationships with clients and client confidentiality.
• Input client data into automated database.
• Conduct outreach activities to recruit clients and re-engage former clients.
• Prepare marketing materials for the program and assist with reports.
• Schedule appointments for clients with referral organizations.
