Job summary

CAMBA in Brooklyn, NY is seeking a Case Manager to conduct intake and assess clients’ needs, maintain documentation, and coordinate services with internal and external CAMBA partners. You will work to help clients set and achieve housing stability goals through ongoing support and referrals.

Qualifications

• Associate’s degree and two years relevant experience or four years of relevant experience.
• Bi-lingual English & Spanish preferred.

Responsibilities

• Conduct initial intake and assessment of clients’ needs and periodic re-assessments.
• Prepare psychosocial evaluation and update according to regulations.
• Obtain and maintain documentation (birth certificate, income proof, medical documents, etc.).
• Collaborate with clients to revise independent living plans and goals.
• Refer clients to internal and external CAMBA services and resources.
• Support clients in overcoming barriers to achieve housing stability.
• Monitor progress with regular face-to-face contacts and update CARES Database.
