Job summary

CAMBA in Brooklyn, NY seeks a Case Manager to deliver contract funded case management and supportive services. The role involves documenting services in Client Track, referring clients to medical, mental health, housing, legal services, and entitlements, and helping clients access benefits.

Responsibilities include maintaining client charts, ensuring medication management adherence, escorting clients to appointments, and conducting home visits.

Qualifications

• Bachelor’s Degree required.
• At least 4 years’ experience in case management or related social service field.
• Experience with housing, homelessness, mental health, or substance abuse is preferred.

Responsibilities

• Provide contract funded case management and supportive services as described.
• Enter all client data into Client Track (assessments, service plans, progress notes).
• Refer and connect clients to medical, mental health, housing, legal services, and entitlements.
• Monitor client progress and adherence to treatment; document via progress notes.
• Coordinate with outside organizations regarding education, healthcare, housing, and legal issues.
• Escort clients to appointments and assist with benefit applications.
• Maintain client files; conduct at least one home visit per month.
• Report repairs to the Housing Specialist and meet monthly projections.
