Job summary

CAMBA in Brooklyn, NY seeks a Peer Navigator to support individuals through HIV/AIDS services, case management and health education. The role emphasizes client confidentiality, universal precautions, and accurate documentation.

You will conduct outreach, maintain client files, and assist in program reporting within a busy community-based setting. Qualifications include an associate or bachelor’s degree or substantial relevant experience, with bilingual Spanish skills preferred.

Qualifications

• Associate or bachelor’s degree with at least 1 year case management experience.
• Alternative: HS diploma with 2+ years’ case management experience.
• Certified Peer Specialist and bilingual Spanish abilities are preferred.
• Strong written and verbal communication; proficient data entry.

Responsibilities

• Maintain client relationships and confidentiality and document progress.
• Input client data and progress into the automated database.
• Prepare reports and assist in planning, coordinating and facilitating events.
• Escort clients to appointments and assist with intake and eligibility.
• Support outreach to recruit clients and re-engage former clients.
