Job summary

PAM Health Rehabilitation Hospital of Greeley is seeking a dedicated Case Manager to coordinate interdisciplinary care for inpatients, review admission records, and determine appropriate levels of care. You will collaborate with therapists, nurses, and social workers to ensure optimal discharge planning and resource use.

Responsibilities include maintaining payor relationships, promoting quality outcomes, and supporting a patient-centered care approach in a fast-paced hospital environment.

Qualifications

• Licensure in the state where the hospital resides is required or preferred.
• Professional disciplines may include OT, PT, RN, RRT, SLP, or LSW.
• Current BLS certification is required.

Responsibilities

• The Case Manager coordinates interdisciplinary care for a designated patient population.
• Reviews inpatient admission records to ensure proper resource utilization and admission level.
• Assesses discharge needs and coordinates the discharge plan.
• Works with the team to achieve financial and quality outcomes.
• Maintains and enhances payor relationships.
