Job summary

PAM Health is seeking a Case Manager to coordinate care across disciplines for a designated patient population. You will review inpatient records to ensure appropriate utilization and determine admission levels of care, while identifying discharge needs and coordinating discharge planning.

Collaborate with the interdisciplinary team to achieve financial and quality outcomes and maintain payor relationships.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care across disciplines for a designated patient population.
• Review inpatient admission records to ensure proper utilization and determine admission level of care.
• Assess discharge needs and coordinate discharge planning.
