Job summary

PAM Health Specialty Hospital of Denver is seeking a Case Manager to coordinate care across inpatient services for a designated patient population. You will review admission records, determine appropriate level of care, assess discharge needs, and coordinate discharge planning with the interdisciplinary team to meet financial and quality goals.

Responsibilities include maintaining payor relationships, promoting a collaborative work environment, and ensuring compliant care delivery.

Qualifications

• State licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
• 3–5 years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

Responsibilities

• The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population.
• Reviews inpatient admission records to ensure proper utilization of hospital resources and determination of admission level of care.
• Assesses discharge needs and coordinates the discharge plan with the interdisciplinary team.
