Job summary

PAM Health in Overland Park, KS is seeking a Case Manager to coordinate care for a designated patient population. You will review inpatient records to ensure appropriate utilization, determine admission level of care, assess discharge needs, and plan discharge in collaboration with the interdisciplinary team.

You will work with the interdisciplinary team to achieve financial and quality outcomes, maintain payor relationships, and support a smooth patient transition.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
• Current BLS certification required.

Responsibilities

• The Case Manager coordinates care for a designated patient population.
• Reviews inpatient admission records for proper utilization and admission level of care.
• Assesses discharge needs and coordinates the discharge plan.
• Collaborates with the interdisciplinary team to achieve financial and quality outcomes.
• Maintains and enhances payor relationships.
