Job summary

PAM Health in Missouri is seeking a Case Manager to coordinate interdisciplinary care for inpatients and to review admission records for appropriate level of care. You will assess discharge needs, collaborate with the care team, and support payor relationships.

Responsibilities include facilitating discharge planning, ensuring resource use efficiency, and contributing to quality outcomes under the direction of the interdisciplinary team.

Qualifications

• Education and Training: Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
• Experience: 3–5 years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

Responsibilities

• Coordinate care across disciplines for designated patient populations.
• Review inpatient admission records to ensure proper utilization and admission decisions.
• Assist with discharge planning and coordinate care to achieve financial and quality outcomes.
