Job summary

PAM Health is seeking a Case Manager to oversee interdisciplinary coordination of care for a designated patient population within our Las Vegas-area hospital. You will review inpatient admission records to ensure appropriate utilization and determine admission level of care, while coordinating discharge planning with the care team.

You will collaborate with physicians, nurses, therapists, and payors to achieve quality and financial outcomes, and maintain payor relationships.

Qualifications

• Education and Training: Licensure in the state where the hospital resides is preferred. Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker. Current BLS certification required.
• Experience: Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

• Coordinate care for designated patient population.
• Review inpatient admission records for proper utilization and level of care determination.
• Assess discharge needs and coordinate discharge plan.
• Collaborate with interdisciplinary team to achieve financial and quality outcomes.
