Job summary

The Whole Child is seeking an ICMS Case Manager in Downey, CA. This full-time field-based role supports families experiencing homelessness and links them to housing, healthcare, education, and community resources.

Responsibilities include coordinating services, maintaining records, assessing needs, and collaborating with healthcare and housing partners to promote stable housing outcomes.

Qualifications

• Bachelor’s degree in social work, psychology, or related field.
• Minimum of three years of direct service experience in social services.
• Knowledge of HMIS and CHAMP systems.

Responsibilities

• Maintain client records in CHAMP and HMIS to ensure data integrity.
• Develop and maintain relationships with housing staff, Housing Authorities, and community partners.
• Conduct comprehensive assessments to identify family strengths, needs, barriers, and service goals.
• Assist families in accessing resources including healthcare, housing supports, childcare, employment resources, and other services.
• Provide individualized service plans to support improved functioning at home, school, and community.
• Complete progress notes within timelines using agency electronic record systems.
