The Whole Child in Downey, CA is seeking a BFH Case Manager to assist families in achieving housing stability and access to essential services. This full-time role focuses on supporting children birth to 18 and their families within a scattered-site model, coordinating with health, education, and social services.

Qualifications

• Bachelor’s degree in social work, psychology, or other mental health related discipline.
• Minimum of 4 years of work experience related to Housing Navigation or rehabilitative services in a mental health setting.

Responsibilities

• Assists families in obtaining needed resources such as health insurance, housing, and low-cost childcare through referrals and linkages.
• Provides individual and group rehabilitation services under various contracts.
• Completes and submits progress notes within 24 hours using HMIS.
• Guides and assists parents/caretakers on managing child behaviors successfully.
• Supports families in identifying internal and external resources to address homelessness, domestic violence, substance abuse, and related stressors.
• Develops and provides client rehabilitation services to improve daily functioning at home, school, and community.
• Provides interagency coordination, referrals, and communication.
