Job summary

The Salvation Army North and Central Illinois Division is seeking a Part-Time, Non-Exempt Case Manager to join the 360 Life Center team in Canton, IL. You will conduct intake, assess needs, develop and monitor case plans, and connect clients to community resources while ensuring compliance with funder standards.

Responsibilities include coordinating referrals, maintaining documentation in HMIS, and supporting housing and supportive services programs.

Qualifications

• Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience may be considered in lieu of a degree.
• A minimum of three to five years knowledge and work experience related to Social Services and supporting others through Case Management is preferred.
• Familiarity with local resources, Continuum of Care and homeless service providers.

Responsibilities

• Conduct client intake interviews to determine eligibility for programs or agency referrals.
• Provide case management services including review of intake material, assessment, direct assistance, case plan development and compliance, referrals to appropriate agencies and transportation arrangements.
• Maintain client records ethically and confidentially; document information in HMIS/SIMS or other required databases.
• Attend monthly Continuum of Care meetings and participate in program administration and reporting.
