• Graduation from an accredited school of professional nursing; BSN preferred or MSW from an accredited program.
• Active Texas RN license or LMSW license in Texas; Case Management certification within 2 years of hire.
• Minimum 3 years nursing or social work experience in acute hospital-based settings or equivalent.

Responsabilidades

• Coordinate patient care progression across the continuum with physicians and the care team.
• Collaborate with physicians, nursing, and other multidisciplinary staff to ensure timely care and discharge.
• Manage discharge planning, utilization management, and documentation per department standards.
• Communicate with payors and third-party entities to facilitate care and reimbursement.
• Lead and participate in clinical performance improvement activities and care pathways.
