Job summary

Memorial Hermann is seeking a Case Manager to support physicians, primary medical homes, and interdisciplinary teams. The role focuses on coordinating patient care, discharge planning, and resource utilization to improve clinical outcomes and patient satisfaction while managing costs.

The position requires solid experience in utilization and discharge planning, with strong communication and leadership skills to work with payors, families, and the care team.

Qualifications

• Graduate of an accredited nursing program; BSN preferred or MSW degree.
• Current Texas RN license or LMSW/LCSW in Texas; case management cert within 2 years.
• 3+ years in nursing or social work in acute hospital settings preferred.
• Experience in utilization management, discharge planning or care management is preferred.
• Strong communication, leadership and organizational skills.

Responsibilities

• Coordinate patient care progression across the continuum with physicians and care teams.
• Collaborate with multidisciplinary teams to ensure timely, cost‑effective care and discharge planning.
• Identify and resolve delays in discharge; streamline pathway to post‑acute resources.
• Utilize quality screens and data to drive performance improvement and patient outcomes.
• Document thoroughly in medical records and coordinate with payors as needed.
• Act as a resource to families, patients and staff for care coordination and planning.
