Responsibilities

• Coordinate and facilitate patient care progression across the continuum.
• Collaborate with physicians, nursing staff and multidisciplinary team to ensure timely care.
• Address barriers to discharge and facilitate appropriate utilization of resources.
• Communicate with payors and care teams to optimize outcomes and costs.
• Lead and participate in performance improvement initiatives and care pathways.
