Job summary

Qualifications

• Graduate of accredited nursing program or MSW program.
• RN license in Texas or LMSW/LCSW in Texas with CM certification within 2 years.
• 3 years nursing or social work in acute hospital or equivalent setting.
• Experience in utilization management, case management, discharge planning, or cost/quality management.
• Strong interpersonal, negotiation and leadership skills.

Responsibilities

• Coordinate patient care across the continuum and with multidisciplinary teams.
• Communicate with physicians, payors, and families to facilitate timely care.
• Identify and resolve delays to discharge and barriers to care.
• Document care plans, progress, and outcomes in patient records and QA systems.
• Lead performance improvement initiatives within case management.
