Job summary

Road To Responsibility, Inc. is seeking a dedicated professional to support and evaluate adults with developmental disabilities in a day habilitation setting.

You will be part of an interdisciplinary team coordinating therapeutic services for each member. Responsibilities include reviewing treatment goals, supervising plans, coordinating OT/PT/Speech, maintaining records, liaising with DDS coordinators and families, and preparing required reports.

Qualifications

• Must have experience with case managing and case reviews in a relevant health care setting.
• Valid Driver’s License.
• Basic knowledge of Microsoft Word and the internet.
• Successful completion of all state and agency mandated trainings.
• Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.

Responsibilities

• Review each recipient’s treatment and habilitation goals monthly and inform staff of any changes in the recipient’s status.
• Supervise the implementation of the ISP/DHSP and maintain the individual’s records.
• Coordinate OT, PT, Speech, Nursing and behavioral services for the assigned caseload of individual members.
• Case Management responsibilities include reviewing or writing monthly progress notes, writing two year Day Habilitation Service Plans, writing assessments and objectives, and completing semi-annual reviews.
• Maintain contact with DDS service coordinators and families of individual members.
• Prepare reports in accordance with state and agency requirements.
• Attend mandated Safety Care training. Responsible for implementing physical restraints on the individuals we support to assist with behavioral management.
