You will review admission records, work with an interdisciplinary team, and maintain payor relationships to support quality and cost outcomes.

Licensure in the state and current BLS are required; eligible disciplines include RN, OT, PT, RRT, SLP, or LSW.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines include OT, PT, RN, RRT, SLP, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care for a designated patient population across inpatient settings.
• Review inpatient admission records to ensure proper utilization and level of care.
• Coordinate discharge planning and collaborate with the interdisciplinary team.
• Maintain and enhance payor relationships.
