Job summary

CARIN Alliance is seeking a Case Management Registered Nurse to coordinate inpatient and observation care from admission to discharge. The role focuses on assessing medical and psychosocial needs to ensure comprehensive care and safe transitions.

Located in Morganton, NC, the RN will identify at-risk patients during transitions and collaborate with physicians, nursing staff and Social Services to develop discharge plans.

Responsibilities

• Coordinate inpatient and observation care from admission to discharge.

• Identify at-risk patients during care transitions and utilize tools like InterQual to monitor progress.
• Collaborate closely with physicians, nursing staff, and Social Services to develop safe discharge plans.
• Arrange post-acute services to support ongoing patient recovery.
• Serve as a liaison for patients and families to facilitate effective communication and continuity of care.
