Rowan Medical Center in Salisbury, NC is seeking an experienced RN for a patient-facing but non-bedside discharge planning role within a high-performing team. You will lead complex discharge planning and work with physicians and MSW case managers to ensure safe, timely transitions.

Collaborating across the care continuum, you will coordinate services for medically complex patients, maintain knowledge of community resources, and uphold accountability for quality discharge planning and post-acute

Formación

• Current RN licensure in the state of employment.
• Associate degree in nursing required; BSN preferred.
• Case management experience preferred; broad bedside care background.

Responsabilidades

• Complex discharge planning
• Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive/complex/safe discharge plans
• Collaborate with MSW case managers for guidance with psychosocially complex cases
• Facilitate coordination of care
• Accept responsibility, authority, and accountability for management of care
• Serve as a resource for discharge planning and care coordination for medically complex cases
• Maintain current knowledge of available community resources/post acute care options
