Mission Hospital seeks a Case Manager RN to support patients and families through the care journey, coordinating services and developing individualized care plans. You will partner with physicians, nurses and department leaders to ensure safe transitions and high-quality care across the continuum.

You will identify at-risk patients, perform comprehensive psychosocial and discharge assessments, and collaborate to optimize transitions with the care team. RN license and BLS required.

Formación

• 2+ years experience in Case Management OR 3+ years experience in a clinical setting
• InterQual experience preferred
• Associates Degree in Nursing
• Licensed Registered Nurse in state of practice
• BLS certification

Responsabilidades

• Provide case management services for inpatient and observation patients as assigned
• Identify patients at risk for adverse outcomes during transition between levels of care
• Perform a comprehensive assessment of psychosocial, medical and discharge needs and available resources
• Coordinate the plan of care and drive discharge planning with the multidisciplinary team and attending physician
• Partner with Social Services to ensure appropriate post-acute medical needs
• Evaluate progression of care using InterQual criteria and escalate progression and transition issues as needed
• Act as liaison with physicians, patient/family, hospital staff and outside agencies
