Qualifications

• Registered Nurse program completion with Florida license.
• Minimum 2 years RN Case Manager experience preferred.
• CPR/ AED/ BLS certified at time of hire or within 30 days.
• Familiarity with hospital discharge planning and post-acute care coordination.

Responsibilities

• Round on patients in hospital and coordinate care with treating providers.
• Document assessments and care plans in EHR.
• Identify post-discharge needs and barriers to care.
• Maintain HIPAA standards and professional communication.
