Job summary

Mission Hospital is hiring a Case Manager RN to support patients and families throughout the care journey. You will coordinate services, develop care plans, and collaborate with physicians, nurses, and leaders to ensure safe transitions and high-quality care.

The role requires RN licensure in the state, 2+ years case management or 3+ years clinical experience, and BLS certification. InterQual experience is preferred. Join a Magnet-designated facility committed to exceptional patient outcomes.

Qualifications

• Required Experience: 2+ years in Case Management OR 3+ years in a clinical setting.
• Preferred Experience: InterQual experience.
• Required Education: Associates Degree in Nursing.
• Preferred Education: Masters Degree in Nursing.
• Required License: Currently licensed as a Registered Nurse in the state of practice.
• Required Certification: BLS

Responsibilities

• Provide case management services for inpatient and observation patients as assigned.
• Identify patients at risk for adverse outcomes during transitions between care settings.
• Perform a comprehensive assessment of psychosocial, medical and discharge needs and resources.
• Coordinate the plan of care and drives the discharge plan by collaborating with the multidisciplinary health care team and physician to facilitate a successful care transition.
• Partner with Social Services to ensure post-acute medical needs and level of care are appropriate.
• Evaluate progression of care using InterQual and approved criteria; escalate progression issues.
• Act as a liaison through effective communications between physicians, patient/family, hospital staff, and outside agencies
