Encompass Health is seeking a Case Management Assistant in Jacksonville to streamline workflow, facilitate communications, and bolster the Case Management department. You will engage with local community resources and help coordinate patient care across teams, contributing to efficient post-acute care delivery.

Qualifications

• Proficiency with Microsoft Office products is preferred.
• Strong typing skills with accuracy and speed.
• Knowledge of medical terminology is preferred.
• Experience with insurance/payor systems is a plus.
• Good verbal and written communication skills.
• Willingness to work flexible hours including weekends/evenings/night shifts if required.
• Available during religious/legal holidays as scheduled.

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather, act on, and maintain Case Management documentation (referrals, admissions, orders, schedules).
• Coordinate daily Case Management Assignment communications.
• Complete pre- and post-discharge activities to inform patients, families and clinicians.
• Gather Quality Improvement and department Performance Improvement data.
• Assist in collecting Key Care Indicators and clinical outcomes data.
• Support regulatory and Joint Commission survey preparedness activities.
