Job summary

Encompass Health Rehabilitation Hospital of Florence is seeking a Case Management Assistant to streamline workflow, facilitate communications, and enhance the efficiency of our Case Management department in Florence, SC. You will engage with local community resources and support patients and clinicians in daily operations.

Qualifications

• High school diploma or equivalent preferred.
• Proficiency in Microsoft Office products.
• Strong typing skills with speed and accuracy.
• Knowledge of medical terminology (preferred).
• Experience with insurance/payor systems (preferred).
• Good visual acuity and communication skills.

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather, take action on, and maintain Case Management documentation such as referrals, admissions, orders, and schedules.
• Effectively coordinate communication of daily Case Management Assignment updates.
• Complete pre- and post-discharge activities to ensure patients, families, and clinicians are aware and prepared.
• Gather Quality Improvement and department Performance Improvement data.
• Assist with collection of Key Care Indicators and clinical outcomes data.
• Assist in regulatory and joint commission survey preparedness activities.
