Encompass Health is seeking a Case Management Assistant to streamline workflow, facilitate communications, and enhance efficiency within the Case Management department. Qualifications

• Proficiency in Microsoft Office applications.
• Fast and accurate typing.
• Knowledge of medical terminology preferred.
• Excellent written and verbal communication skills.
• Able to adapt to varying schedules including weekends/evenings.

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather, action on, and maintain Case Management documentation (referrals, admissions, orders, schedules).
• Coordinate communication of daily Case Management Assignment updates.
• Complete pre- and post-discharge activities to inform patients, families, and clinicians.
• Collect Quality Improvement and department Performance Improvement data.
• Assist in gathering Key Care Indicators and clinical outcomes data.
• Support regulatory and Joint Commission survey preparedness activities.
