Job summary

Encompass Health seeks a Case Management Assistant to streamline workflow, facilitate communications, and enhance Case Management efficiency. You will engage with local resources and support care coordination across patients and families in the post-acute setting.

The role emphasizes teamwork, documentation accuracy, and participation in regulatory readiness, with opportunities for growth within a national inpatient rehabilitation leader.

Qualifications

• High school diploma or equivalent preferred.
• Proficiency in Microsoft Office products.
• Strong typing skills with speed and accuracy.
• Knowledge of medical terminology (preferred).
• Experience with insurance/payor systems (preferred).
• Good visual acuity and communication skills.
• Flexibility to work weekdays/weekends, evenings, or night shifts if necessary.
• Availability during religious/legal holidays as scheduled.

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather, take action on, and maintain Case Management documentation (referrals, admissions, orders, schedules).
• Coordinate communication of daily Case Management Assignment updates.
• Complete pre- and post-discharge activities to inform patients, families, and clinicians.
• Gather Quality Improvement and department Performance Improvement data.
• Assist in collecting Key Care Indicators and clinical outcomes data.
• Assist in regulatory and Joint Commission survey preparedness activities.
