Job summary

Encompass Health in Anderson, SC, seeks a dedicated Case Manager to coordinate patient care from admission to post-discharge, collaborating with an interdisciplinary team to develop and monitor plans of care.

You will assess patient needs, facilitate timely discharges, ensure regulatory compliance, and contribute to quality outcomes while supporting families and communities through evidence-based rehabilitation. Experience in case management and CCM/ACM encouraged.

Qualifications

• Must possess an assessment capability within the practitioner's scope.
• CCM or ACM certification required or must be obtained within two years of hire.
• 2 years of rehabilitation experience preferred.

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs.
• Coordinate tentative discharge plans and contingency plans with the team.
• Participate in discharge planning and execution of the patient discharge experience.
• Monitor patient experience across quality, timeliness, service, payors, and expectations.
• Lead weekly team conferences and adjust treatment plans as needed.
• Complete case management addendums and required documentation.
• Maintain knowledge of regulations, policies, and department operations.
• Review case management reports and plan actions based on data.
• Understand payor requirements and recertification needs.
• Attend ACT meetings to identify trends and reduce transfers.
