Encompass Health in Bluffton, SC is seeking a Case Management Assistant to streamline workflow, facilitate communications, and support the Case Management team in coordinating patient care.

You will interact with local resources, manage referrals, admissions, orders, and schedules, and help ensure smooth discharge planning while upholding our commitment to compassionate, patient-centered care.

Qualifications

• High school diploma or equivalent preferred.
• Proficiency in Microsoft Office products.
• Typing speed and accuracy required.
• Knowledge of medical terminology preferred.
• Experience with insurance/payor systems preferred.
• Good visual acuity and communication skills.
• Flexibility for various shifts and holiday availability.

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather, take action on, and maintain Case Management documentation such as referrals, admissions, orders, and schedules.
• Effectively coordinate communication of daily Case Management Assignment updates.
• Complete pre- and post-discharge activities to ensure patients, families, and clinicians are aware and prepared.
• Gather Quality Improvement and department Performance Improvement data.
• Assist with collection of Key Care Indicators and clinical outcomes data.
• Assist in regulatory and joint commission survey preparedness activities.
