Job summary

Volunteers of America Ohio & Indiana seeks a Case Coordinator to support veterans in a residential facility. You will assess needs, coordinate services, and promote stability through individualized service plans based on each veteran's goals.

You will connect veterans to housing, employment, and income support and document progress with care coordination. Collaborate with an interdisciplinary team and community partners while transporting veterans to required appointments and maintaining timely

Qualifications

Associates degree or higher with experience in social services.
Valid driver's license required.
Bachelor's degree is a plus for consideration.

Responsibilities

Provide direct support and case management services to veterans residing in a residential facility.
Assess veteran needs and develop individualized service and goal plans.
Assist veterans with housing, employment, and income support services.
Connect veterans with community resources and benefits.
Provide ongoing coordination and monitor progress toward goals.
Maintain accurate documentation including case notes and service plans.
Collaborate with internal teams and external partners to coordinate services.
Transport veterans to appointments and activities as needed.
Maintain regular communication with veterans to monitor progress and barriers.
