Job summary

Hartford HealthCare Independence at Home is recruiting a Homemaker Caregiver to support elderly and disabled clients in their homes. The role includes personal care, meal preparation, light housekeeping, and coordinated communication with nurses and coordinators across Hartford HealthCare’s Connecticut network.

You will document care plans, participate in case conferences, and provide emotional support to clients and families while maintaining professional conduct and high-quality service.

Qualifications

• Must have a driver's license and car with auto insurance.
• One year of previous personal care experience at a facility or agency.
• Two years of continuous work experience.
• New Grad CNA's are welcome.
• Comfortable driving within 30 minutes of home for cases.

Responsibilities

• Personal hygiene and grooming care to clients.
• Assistance with toileting, ambulation, etc.
• Providing meals for clients' nutritional needs.
• Reporting changes to nurse or supervisor.
• Performing routine housekeeping and home management duties.
• Recording care on the care plan and submitting timely.
• Assisting with emotional support for clients and families.
• Assisting with routine therapy exercises.
• Participating in case conferences as requested.
• Attending required in-service education sessions.
• Using computer software and smartphone for education.
• Representing the company with a high level of customer service.
• Thinking critically without direct supervision and expanding when appropriate.

Shift Detail: Hourly cases range anywhere from 4- 12 hours at any time in the day, evening or night. Shifts will take place within 30 minutes of where you reside.

The Homemaker Caregiver Position Provides

• Attends all required in-service education sessions
• Has the ability to use computer software program and smart phone for required education
• Represents the company in a positive and professional manner through a high level of customer service
• Can think critically without direct supervision and the ability to expand when appropriate.
