Job summary

Hartford HealthCare Independence at Home is seeking a Homemaker Caregiver to provide in-home personal care and support services in the greater Hartford area. You will assist clients with hygiene, mobility, meals, and daily tasks while reporting changes to nursing staff.

Qualifications

• One year of previous personal care experience at a facility or agency.
• Two years of continuous work experience.

Responsibilities

• Assist with personal hygiene and grooming for clients.
• Help with toileting, ambulation, and daily living tasks.
• Prepare meals to meet clients' nutritional needs.
• Report changes in clients' condition to nurse or supervisor.
• Perform routine housekeeping and home management duties.
• Record care provided on the care plan and submit promptly.
• Provide emotional support to clients and families.
• Assist with routine therapeutic exercises as directed.
• Attend case conferences as requested and participate in education.

Shift Detail: Hourly cases range anywhere from 4- 12 hours at any time in the day, evening or night. Shifts will take place within 30 minutes of where you reside.

The Homemaker Caregiver Position Provides

• Personal hygiene and grooming care to clients
• Assistance with toileting, ambulation, etc.
• Providing meals for clients' nutritional needs
• Reporting changes in clients' condition to nurse, supervisor, or client care coordinator
• Performing routine housekeeping and home management duties
• Recording each client's care on care plan and submitting care plan in a timely manner
• Assisting with providing emotional support for the clients and their families
• Assisting with routine therapy exercises
• Participating in case conferences as requested
• Attends all required in-service education sessions
• Has the ability to use computer software program and smart phone for required education
• Represents the company in a positive and professional manner through a high level of customer service
• Can think critically without direct supervision and the ability to escape when appropriate.

Qualifications

Hourly Caregivers must have a driver's license and car with auto insurance
One year of previous personal care experience at a facility or agency
Two years of continuous work experience
New Grad CNA's are welcome
Caregivers must be comfortable driving within 30 minutes of where they live for cases.
