Job summary

Enhabit Home Health & Hospice is seeking dedicated nurses, therapists, or social workers to support post-acute care transitions. You will guide patients and families through care plans, coordinate with health systems, and promote high-quality outcomes in home-based settings.

The role requires clinical licensure and at least two years of field experience, with strong knowledge of regulatory guidelines and technology usage for mobile devices.

Qualifications

• Must have two years of demonstrated field experience.
• Must understand federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice operations.
• Basic demonstrated technology skills including operation of a mobile device.

Responsibilities

• Assist patients in navigating post-acute care to improve outcomes and referral source satisfaction.
• Develop transition plans that are patient-centered, promoting quality and efficiency.
• Represent the area branches in strategic relationships with health systems, hospitals, and physicians.
