Qualifications

• Must be a graduate of an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Must have two years' demonstrated field experience.
• Must have knowledge of federal, state, and local laws governing Medicare certified home health and hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Assist patients in navigating post-acute care to improve outcomes.
• Develop transition plans that are patient-centered and promote quality and efficiency.
• Represent Enhabit in relationships with health systems, hospitals, physicians and leaders beyond the local team.
