Job summary

Enhabit Home Health & Hospice in Atlanta, GA seeks a qualified health professional to support post‑acute care transitions and improve patient outcomes across home-based services. You will work with branches, health systems and physicians to promote quality care and compliant practices.

The role requires a degree in nursing, therapy or social work, state licensure, and two years of field experience, plus basic technology skills for mobile devices.

Qualifications

• Must graduate from an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Must have two years' demonstrated field experience.
• Must understand federal, state, and local laws and regulatory guidelines governing Medicare home health and hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Assist patients in navigating post-acute care with the goal of improving outcomes and referral source satisfaction.
• Develop transition plans using evidence-based guidelines to promote quality and efficiency in home care.
• Represent the organization in strategic relationships with health systems, hospitals, physicians and executive level opportunities.

Education

Graduate of nursing, therapy or social work school

Licensed in practicing state

Two years' field experience

Knowledge of Medicare rules and regulations

Basic technology skills (mobile devices)
