Qualifications

• Must be graduate of an approved nursing, therapy or social work program.
• Must be licensed in the practicing state.
• Must have two years of field experience.
• Knowledge of federal, state and local laws governing Medicare home health and hospice.
• Basic technology skills, including use of a mobile device.

Responsibilities

• Assist patients in navigating post-acute care to improve outcomes and referral source satisfaction.
• Integrate evidence-based guidelines, prevention protocols and other metrics in transition plans.
• Represent the area branches in relationships with health systems and hospitals at executive levels.
