Job summary

Enhabit Home Health & Hospice seeks professionals dedicated to improving patient outcomes through careful navigation of post-acute care. You will develop transition plans grounded in evidence-based guidelines and collaborate with health systems, hospitals, and physician groups to ensure high-quality care and efficient service delivery.

The role requires a nursing, therapy, or social work background with active licensure and at least two years of experience in field settings.

Qualifications

• Must be a graduate of an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Must have two years' demonstrated field experience.
• Must have demonstrated understanding of federal, state, and local laws governing Medicare home health and hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Assists patients in navigating post-acute care to positively impact outcomes and referral satisfaction.
• Develops patient-centered transition plans using evidence-based guidelines and protocols.
• Represents the organization in strategic relationships with health systems, hospitals, physicians, and executives.
