Qualifications

• Graduate of an approved nursing, therapy, or social work program.
• Licensed in the state where practicing.
• Two years of demonstrated field experience.
• Understanding of federal, state, and local laws governing Medicare home health and hospice.
• Basic demonstrated technology skills, including use of a mobile device.

Responsibilities

• Assist patients navigating post-acute care to improve outcomes and satisfaction.
• Develop transition plans using guidelines to promote quality and efficiency.
• Represent branches in strategic relationships with health systems and physicians.

Education

Graduate of an approved nursing/therapy/social work program
