Qualifications

• Minimum 1 year in home health or hospital-based case management.
• RN/LPN/SW/PT licensure required.
• RT certification preferred.
• Valid driver's license and reliable transportation required.
• Knowledge of Medicare/insurance guidelines.

Responsibilities

• Achieve monthly production goals and MC admission targets for assigned locations.
• Coordinate care plans and arrange services including DME and infusion.
• Educate patients on post-discharge needs and coordinate physician follow-up.
• Maintain documentation and communicate with referral sources and leadership.
