Job summary

Care New England Health System in Rhode Island seeks a Home Health (HH) clinician to manage a caseload from admission to transition into the community, coordinating interdisciplinary support and referrals when appropriate.

The clinician will monitor and adjust treatment to achieve positive therapeutic outcomes and identify opportunities for process improvement in accordance with organizational policies and regulatory standards.

Qualifications

• Equivalent education required for RI licensure.
• Bachelor's Degree (min) in relevant discipline strongly preferred.
• A minimum of one (1) year progressive clinical experience required.
• Previous experience in an acute care or hospice environment setting preferred.
• Valid CPR certification required.
• Valid RI Registered Nurse license
• Valid driver’s license and automobile liability insurance are required.
• Ability to read and interpret documents such as medical records, medical reports, etc.
• Must be able to speak, read, and write English with excellent oral and written communication skills required.
• Ability to define problems, collect data, establish facts and draw valid conclusions.
• Ability to interpret an extensive variety of technical instructions furnished in written, oral, diagram or schedule form.
• Must be able to deal with several abstract and concrete variables.
