Care New England Health System is seeking a field clinician in Warwick, RI to manage a caseload from admission to community transition. You will coordinate interdisciplinary care, monitor treatment, and refer patients to appropriate services, ensuring compliance with state, federal, and accreditation standards.

Formación

• Equivalent education required for RI licensure.
• Bachelor’s Degree (min) in relevant discipline strongly preferred.
• Valid RI Registered Nurse license is required.
• Valid driver’s license and automobile liability insurance are required.

• Manage a caseload of patients from admission to transition into the community.
• Coordinate interdisciplinary care and referrals when appropriate.
