Job summary

Care New England seeks a field clinician to manage a caseload from admission through community transition. You will coordinate interdisciplinary care and referrals, ensuring treatment aligns with patient needs and safety standards.

Under supervision of the Team/Program Manager, you will monitor and adjust treatment to drive positive outcomes and contribute to process improvements in accordance with policies and regulations.

Qualifications

• RN licensure (RI) and CPR certification required.
• Bachelor’s degree (minimum) in a relevant field preferred.
• At least 1 year of progressive clinical experience; acute care or home health experience preferred.
• Valid driver’s license and automobile liability insurance required.
• Strong written and spoken English with excellent communication skills.
• Ability to read, interpret medical records and write reports and procedures.

Responsibilities

• Manage a caseload from admission through transition to community.
• Coordinate interdisciplinary care and referrals as appropriate.
• Monitor and adjust treatment to achieve positive therapeutic outcomes.
