Job summary

Care New England seeks a Transition Manager to work with a group of physicians to drive cost-effective, quality-focused care interventions. You will maintain ongoing communication with patients, care coordinators, social workers and physicians to ensure resources and support in the community.

You will track patients along the continuum of care and help develop plans to improve outcomes. The role requires an RN with a RI license and BSN, at least five years in acute care or case management, and

Qualifications

• Registered Nurse with a RI license and BSN or equivalent.
• Minimum 5 years of clinical Acute Care or Case Management experience.
• Knowledge of utilization review, QA, discharge planning and payor regulations.
• Strong written and verbal communication within a team.

Responsibilities

• Collaborate with physicians to pursue cost-effective, quality-focused management interventions.
• Maintain ongoing communication with patients, coordinators, social workers and physicians.
• Track patients along continuum of care and develop improvement plans.
• Document promptly in Epic and Cerner systems.
• Support discharge planning and patient flow through the care team.
• Identify patients for case management and social service referrals.
• Engage in process improvement to reduce admissions and readmissions.
