VNA Home Health & Hospice in Rhode Island is seeking an RN to manage a patient caseload from admission to transition into the community, coordinating interdisciplinary care and referrals when appropriate.

You will monitor and adjust treatment to achieve positive therapeutic outcomes and identify opportunities for process improvement, in line with organizational policies and accreditation standards.

Qualifications

• RI RN license required for licensure in Rhode Island.
• Bachelor's degree in a relevant discipline strongly preferred.
• Minimum of 1 year progressive clinical experience; acute care or hospice preferred.
• Valid CPR certification required.

Responsabilités

• Manage or care for a caseload of patients from admission to transition into the community.
