Job summary

Care New England Health System is seeking a field clinician to manage a caseload from admission to community transition, coordinating interdisciplinary care and ensuring referrals are made when appropriate. You will monitor and adjust treatment to achieve positive outcomes while aligning with organizational policies and standards.

Equivalent RI licensure and a Bachelor’s degree are required, with a strong preference for related discipline experience.

Qualifications

• Equivalent education required for RI licensure
• Bachelor’s Degree (min) in relevant discipline strongly preferred.

Responsibilities

• Manage a caseload of patients from admission to transition into the community.
• Coordinate interdisciplinary care and support referrals as appropriate.
• Monitor and adjust treatment to achieve positive therapeutic outcomes.
• Identify opportunities for process improvement in accordance with policies and regulations.
