Job summary

Care New England is seeking an RN Case Manager (RN CM) in Rhode Island to coordinate clinical care across the continuum, emphasizing quality outcomes and cost-effective care. Qualifications

• Must be licensed as a Registered Nurse in Rhode Island.
• Minimum of 3–5 years related clinical and/or Case Management experience.
• Certification in Case Management (CCM or ACM) earned within 24 months of employment (part-time prorated).
• Ability to effectively communicate with physicians and interdisciplinary care team to achieve resolutions.
• Knowledge of level of care management, quality management, discharge planning.
• Proficiency with multiple computer systems (EMR, Microsoft Office).

Responsibilities

• Coordinate clinical care for a designated patient population across the continuum of care.
• Engage in transition of care planning and discharge planning.
• Perform utilization review for POE/ED RN Case Manager responsibilities.
• Allocate resources for POE/ED RN Case Manager assignments.
• Ensure quality, compliant care and documentation across departments.
• Participate in weekend and holiday coverage rotation.
